| State | Providers | Lowest | 25th pct | Median | 75th pct | Highest | Medicare pays | Median vs Medicare |
|---|---|---|---|---|---|---|---|---|
| Florida | 2 | $6.08 | $7.10 | $8.11 | $9.13 | $10.14 | $8.68 | 0.9x Medicare |
| Georgia | 1 | $10.14 | $10.14 | $10.14 | $10.14 | $10.14 | $8.68 | 1.2x Medicare |
| Illinois | 1 | $10.14 | $10.14 | $10.14 | $10.14 | $10.14 | $8.68 | 1.2x Medicare |
| Ohio | 5 | $5.90 | $5.90 | $6.08 | $6.94 | $17.53 | $8.68 | 0.7x Medicare |
| Pennsylvania | 2 | $10.14 | $10.14 | $10.14 | $10.14 | $10.14 | $8.68 | 1.2x Medicare |
| Virginia | 1 | $10.14 | $10.14 | $10.14 | $10.14 | $10.14 | $8.68 | 1.2x Medicare |

**“x Medicare”** compares each price with what original Medicare pays for the same lab test under its clinical laboratory fee schedule (26CLABQ4, in force from 2026-10-01): one national price, the same in every state. Insurers write their contracts as a share of Medicare, so 1.3x means the insurer pays 30% more than Medicare would; Medicare Advantage plans generally pay close to 1.0x.

Prices are what the insurer has agreed to pay, not what you will be billed. This is a statewide summary: prices vary far more between towns and between practices than these medians suggest, and "providers" counts everyone with a contracted price, not only those who perform the procedure.
